Squash Section - Accident & Incident Reporting


 

The Squash Section has the following policy on reporting ACCIDENTS, INCIDENTS & INJURIES that occur on the squash court.

There is also a web page that should be read by all squash players that gives clear advice on safety. This is with regard to the wearing of eye protection (goggles), asking for ‘lets’ and first aid.    

 

·    Please fill in the details in the EXCEL LOG FILE
The file lives on the Squash External Disk
In directory

     \03 Squash Files
\04 Squash Documents
\10 Admin Files Docs Xls Ppt
\Squash Accident Incident and Injury Log File 24.11.08.xls (password protected for confidentiality)

 

     The responsibility for filling in this log lies with one of the Officers of the Squash Club Management Committee.

 

·    Send details of all accidents, incidents and injuries to:
The Insurance Company - Perkins Slade (latest address on Insurance web page and email in the Excel file cited)
The Institute (ICR) / RMH Site Management or Safety Departments dependent on their request.
The ICR has a safety reporting system called RIVO

There are potential data protection issues associated with both these routes.
The Excel Squash Accident Log has therefore been password protected.
The password is known by the Officers of the Squash Club Management Committee
